Does olive oil break a fast?
Olive oil breaks a fast on calories — roughly 120 per tablespoon — but it contains no protein and no carbohydrate, so it does not raise insulin and does not interrupt ketosis. As with MCT oil, the verdict depends entirely on whether your fast is about energy or about carbohydrate.

Why
Olive oil is essentially pure triglyceride, so it supplies roughly 9 calories per gram with no amino acids and no carbohydrate. That is why it ends a caloric fast while leaving ketosis intact. It is not metabolically silent in other respects: extra-virgin olive oil has gastrointestinal effects associated with lower postprandial glycaemia in type 1 diabetes, food fried in extra-virgin olive oil improved postprandial insulin response in obese insulin-resistant women, and a controlled comparison of palm olein, olive oil and lard found differing effects on postprandial lipaemia with no effect on plasma glucose or insulin. Those are meal studies, and the mechanism running through them is delayed gastric emptying rather than anything that helps a faster. A tablespoon of olive oil during a fast is a hundred and twenty calories with no compensating benefit.

The exceptions
This is usually where the real question lives.
- The ketosis exception is real — olive oil is a staple of ketogenic eating and will not interrupt ketone production.
- A "shot" of olive oil is not a health protocol with fasting support. It is 120 kcal.
- Olive oil aids absorption of fat-soluble vitamins, which is a reason to take them together at a meal.
- For a lipid panel, fat intake before the test is exactly what the fast was designed to exclude.
